PET/CT plays an increasingly important role in assessing treatment response for diffuse large B-cell lymphoma (DLBCL). But, how can initial PET data be used to improve estimates of prognosis for patients with DLBCL? This study used data from >1000 patients from 5 trials to generate a prognostic index based on baseline PET/CT data and compared performance with the International Prognostic Index (IPI). They calculated the metabolic tumor volume (MTV) as the volume of tumor with a standardized uptake value ≥4.0 using ACCURATE or MIM software. Ultimately, a model incorporating MTV, age, and stage performed better at predicting survival than the IPI and also performed better at identifying high risk patients. The International Metabolic Prognostic Index (IMPI) is more complex than the IPI to calculate, but a strength is that it can give a continuous, individualized estimate of the risk of progression or death.  | Mikhaeel J Clin Oncol 2022
