Mastectomy is the preferred surgical approach for local-only breast cancer recurrence after prior breast conservation and radiation. But could repeat breast conservation be an option? RTOG 1014 was a small phase 2 trial that showed such an approach may have acceptable toxicity and disease-control. Here is a look at data from over 1300 patients offered mastectomy or breast conservation and brachytherapy at 15 centers in 7 European countries. Most (71%) were treated with mastectomy. Using a propensity-score matched comparison of mastectomy and breast conservation, they found no significant difference in the rate of cumulative 3rd breast events over 5 years (2.3 vs 2.8%).
