Can low grade upper tract urothelial carcinoma (UTUC) be treated with anything less radical than radical nephroureterectomy? The OLYMPUS trial explored the use of a mitomycin gel via retrograde catheter for non-surgical treatment of 0.5-1.5 cm low grade UTUC. The mitomycin gel was administered once weekly for 6 treatments. At 4 to 6 weeks after treatment, an impressive 59% of patients had complete response. However, serious adverse events occurred in 27%. Frequent (>30%) side effects included ureteral stenosis, UTI, hematuria, and pain.
