Cholangiocarcinoma in which biliary site is a standard candidate for definitive liver transplantation? If you said the obvious answer of in the liver, you’re wrong. Hilar cholangio is currently the only site with liver transplantation offered as standard treatment for those responding well to neoadjuvant therapy. A case series of 12 patients enrolled in a prospective single-center (a Houston Methodist transplant center, to be exact) protocol demonstrates exciting results with liver transplantation for locally-advanced unresectable intrahepatic cholangio. All patients received neoadjuvant gemcitabine-based chemo and 6 went on to receive a transplant.
